主持人安珀·史密斯: 锡拉丘兹上州医科大学, 纽约邀请你成为“知情病人”, with the podcast that features experts from Central 纽约's only academic medical center.
纤维肌痛无法治愈, 但治疗这种疾病是可行的, which is characterized by widespread musculoskeletal pain with 疲劳和睡眠, 记忆和情绪障碍.
Hensel.
And like you mentioned on the introduction, it usually has a couple features to it.
It typically is something that's present for months or years, and it's an ongoing process. It tends to develop in middle adulthood to later adulthood when it first develops,
We don't have any tests or any imaging or anything that can really 说 是的 or no to fibromyalgia.
That means it can affect different people in pretty substantially different ways.
You can have vague, diffuse feelings of aching, feelings of soreness, at those sites. 真的, the rule with fibromyalgia is that you have these diffuse symptoms that can be all over the body, 但他们来了又走.
主持人安珀·史密斯: Are there other 条件 that someone with fibromyalgia might also have, other medical 条件?
我们有血液测试可以告诉我们, 是的, 你的身体发炎了, but is that because of fibromyalgia or is that because you have rheumatoid arthritis? So we can't use those types of blood tests to 说 it's fibro without anything else.
There oftentimes isn't a clear and absolute picture with a disorder like fibromyalgia. 有几个工具, 临床调查问卷, 基本上, 问一系列问题,再问一系列问题, 种, of steps to go through for the medical provider to 种 arrange how things are presenting.
And those 临床调查问卷 have a score that's associated with them, 然后把答案加起来. Some of the older types, they then have to multiply and do some other things.
主持人安珀·史密斯: Are there 条件 that you have to rule out that maybe look like fibromyalgia, and you want to take care and make sure that it's not one of those things?
Medicine in general has moved more and more away from the rule-out 条件, and part of that's because you can have fibromyalgia on top of rheumatoid arthritis.
如果你有风湿性关节炎, 我们在那之后推动我们的治疗, and we just ignore the possibility of fibro while we're not comprehensively managing what your clinical situation is.
Hensel.
There's a couple ideas, a couple theories, as to what it is and what causes it.
But some of those things are also situational or lifestyle or driven by exposures.
具体原因,我们还不清楚. there is a theory that it has something to do with the estrogenic access or the pro-inflammatory nature.
There's a lot of theories, but I don't think there's a definitive answer to that yet.
医学博士亚历克斯·亨塞尔: 这是一件悬而未决的事情, 我会说, because it is a condition that we are figuring a bit more out about fundamentally what it is.
It was 虽然t that it was really driven by the body attacking itself as an autoimmune process.
I'd argue -- I have a little bias because I'm a family medicine provider -- but I'd argue that it's a good fit for primary care to tackle. 但如果我们到了超出范围的地步, 什么情况比初级保健所能处理的更极端, rheumatology is still in the scope; they're one specialty we could talk to.
我们知道有几个探员工作得很好, and research is ongoing for other ways to use those or other approaches to them.
The medicines that seem to have the best effect are actually the antidepressant and anti-seizure class medicines. 尤其是抗抑郁药, a class of medicines we call the SNRIs; those are the serotonergic (and) noradrenergic reuptake inhibitors. 我们并不完全明白其中的原因, but modifying the way that the brain signals respond changes how it perceives pain signals with these medicines.
That one is a fairly widely studied medicine that does have a benefit on fibromyalgia.
医学博士亚历克斯·亨塞尔: 是的, this was actually something I came across when I was doing some research into this subject a little while back.
That actually helps people to experience less pain and less lifestyle 乏力。 and other lifestyle impacts.
The lifestyle modification that's the most studied and seems to have the biggest impact, 有规律的体育活动不需要特别剧烈, but something that increases the metabolic activity of the muscles and increases the strain on the joints, 不过分.
主持人安珀·史密斯: 好吧, let's talk about what life is like for someone who has fibromyalgia.
在某些情况下, it's something in the background that people just 种 muscle through or push through to live the life that they want to live. In other cases, it can be quite debilitating and can interfere with every activity of daily life.
It can be a progressive condition where things can get slowly worse as time goes on,
It's progressive; it's slow. 它也有起有落, 你有一系列的症状吗, 你正在经历的事情的恶化, but then that can resolve over the course of a few weeks and get a little bit better.
What sort of long-term outlook do you see for people who have fibromyalgia?
That also varies In most cases, it's not something that you're going to cure. 药物, 药物, supplemental approaches: Those do have an impact on disease progression and burden, 但他们往往有更多的时间限制.
