Your Appointment

Appointments are primarily made by physician request for consultation. Self referrals are accepted with proper supporting clinical records and for consultation purposes.

Reminder calls are made by the clerical staff 23 days in advance. You will also be contacted by pre-registration 23 days before your appointment and a fast pass number will be given to you to bring to your appointment.

If you need to cancel an appointment please give our office 24 hour notice so we may offer the appointment to another patient who may need it. We would be happy to reschedule your appointment.

New Patients

All new patients are requested to bring:

New patient consults should be prepared to spend a minimum of 90 minutes in the office for an office visit which includes preparation, physician time, and discharge time.
